| Chiropractic care is covered under many insurance plans. As a courtesy to you we will submit your claims directly to your insurance carrier. In order for us to properly verify benefits and/or submit to your insurance, we will need specific information from you which is listed on our Benefits Assessment form. Please fill out the form in its entirety so that there will not be any delays in the claim processing procedure. Our recommendations are based on a desire to see you get well and stay well. Regardless of your coverage, we’ll suggest the chiropractic care we think you need. If you do not have any insurance coverage, we gladly accept cash, checks, Master Card, and Visa.
